Our purpose was to develop and test a new Internet-based decisions aid for an existing computer module, the Dementia Caregiver Module, to help caregivers successfully implement a decision regarding care.

The Older Americans Act of 1965 (OAA) was enacted to help older individuals remain in their homes and communities as long as possible. Based on formulas specified in law, the Department of Health and Human Services' (HHS) Administration on Aging (AOA) awards grants to fund services for older individuals. State and local agencies are responsible for planning, developing, and coordinating services within each state, as well as distributing OAA grant funds to local Area Agencies on Aging (AAA) through intrastate funding formulas developed by each state in accordance with OAA requirements. Services funded by titles III and VII OAA grants, as well as by other sources, provide a broad range of vital assistance to older adults and include congregate and home-delivered meals, home-based care, transportation services, and elder abuse, neglect, and exploitation prevention programs. In fiscal year 2012 approximately $1.4 billion and $22 million, respectively, was provided for services and programs under title III and title VII of the OAA. In fiscal year 2010--the most recent year for which data are available--these funds were used to serve nearly 11 million older Americans and their caregivers. While all adults age 60 and over are eligible for services, the OAA title III requires programs to target or prioritize service to older individuals with the greatest economic and social need. The OAA defines such need as (1) living below the poverty threshold; (2) having physical or mental disabilities that pose risk for institutional placement; or (3) cultural, social, or geographic isolation, including isolation caused by language, race, or ethnic status.

The United States has been at war longer than any time in its history. While thousands have been wounded in these conflicts, advances in battlefield medicine mean many of our troops survive catastrophic wounds. The nature of many of their wounds, however, means some require long-term caregiving support. The Elizabeth Dole Foundation commissioned RAND Corp to assess the needs of military caregivers, scan the services available to them, and identify how their needs are -- and are not -- being met. This report reviews existing research on the needs of caregivers in general, and assesses how lessons learned can be applied to military caregivers. We also present information gleaned from military caregivers themselves and from policymakers and program officials who either directly support, or advocate on behalf of, military caregivers. We provide a snapshot of the number and characteristics of military caregivers, the roles they serve, the physical and emotional impact caregiving has on their lives, and the resources available to them. Military caregivers tend to be younger women with dependent-age children, dealing with a different set of patient variables than the general caregiver population. Along with typical caregiver responsibilities, military caregivers also act as case managers navigating multiple health systems, advocates for new treatment, and financial and legal representatives. Many are also raising children and holding jobs outside the home. Studies indicate that caregivers in general suffer from physical strain and overall worse health and tend to put their own concerns behind those of the individuals for whom they are caring. Military caregivers suffer disproportionately from mental health problems and emotional distress. Many government programs are still in their infancy, and community resources are scattered and uncoordinated. Difficulties are presented by differing eligibility criteria, lack of access, and the way caregivers' needs change over time.

Eldercare is an experience that for all it's 'universal' quality is unique for every family. We have learned that families often don't know where to turn or what kind of assistance is available when an older family member suffers a significant mental or physical decline. This notebook is intended as a resource for family members who are involved in caring for older adults in North Carolina. Its purpose is to help the family caregiving experience to be as successful as possible by providing basic information on caregiving, normal aging changes, and community services which might be partners in caring for a loved one. The 'language' of caregiving (terms that are used by professionals) will be introduced as well as the kinds of services that are available in many communities in the state to help caregiving families.

Results are presented of a project undertaken to implement and assess family support services that were aimed at enhancing the ability and willingness of families to retain the primary responsibility for caring for aged family members. An experimental design was used to assess the impact of family support programs that provided family training in conjunction with volunteer respite services. The family units consisted of elderly persons who were being assisted by a family member and at least one family member who was providing regular care or assistance. The findings suggest that one of the major challenges of service providers is locating families early enough to help them with their caregiving tasks. Data indicate also that caregivers who were spouses and received respite benefits reported lower levels of objective burden. Findings suggest that a volunteer program is likely to be successful only when it is designed to augment a paid respite program. Program developers are also warned against underestimating the amount of professional staff time that is necessary to operate a volunteer program.

A major finding is that the needs of the Military Family Support Service Center staff are for basic information on how to identify and locate aging services. The second major finding is that they perferred to receive training materials and information that can be used in individual self-directed training rather than having the project staff do training sessions for them.
